Terms of Service
Effective date: August 25, 2026 · Applies to remeditag.com, app.remeditag.com, and the RemediTag API/CLI (the “Service”)
1. Acceptance of Terms
These Terms of Service (“Terms”) govern access to and use of RemediTag, provided by CCM Bridge Inc., a Minnesota corporation (“CCM Bridge,” “we,” “us”). By creating an account or using the Service, you and the organization you represent agree to these Terms.
2. Eligibility
The Service is intended for business use by organizations and professionals operating in Customer Communications Management, healthcare payer operations, and related fields. You must be at least 18 and authorized to bind your organization to these Terms.
3. Account Terms
You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Notify us promptly of any unauthorized use.
4. Acceptable Use: Document Scope
RemediTag is designed to process plan-level regulatory and template documents (for example: ANOC, EOC, Summary of Benefits, Formulary, EOB, and NOA/NDN documents in template or plan-level form). You agree not to upload, and RemediTag is not intended to process, any document containing individual member or beneficiary protected health information (PHI) or other data that would make CCM Bridge a HIPAA business associate. You are responsible for ensuring documents you upload comply with this restriction. You also agree not to use the Service to violate any law, infringe intellectual property rights, or interfere with the Service’s operation or security.
5. Subscription, Fees, and Payment
The Service is billed as an annual subscription per the plan tier selected at signup (Regional, Mid, Large, or a custom Enterprise agreement). Fees are processed via Stripe and are non-refundable except as required by law or as separately agreed in an Enterprise contract. Subscriptions renew automatically at the then-current rate unless cancelled before the renewal date.
6. Intellectual Property
CCM Bridge owns all rights in the Service, including its software, remediation engine, and branding. These Terms grant you a limited, non-exclusive, non-transferable right to access and use the Service during your subscription term for your internal business purposes.
7. Your Content
You retain ownership of the documents you upload and the corrected outputs generated for you (“Your Content”). You grant CCM Bridge a limited license to process, store, and transmit Your Content solely to provide the Service to you. We do not use Your Content to train models for other customers or claim ownership of it.
8. Warranties and Disclaimers
The Service is provided “as is.” While RemediTag is designed to improve PDF/UA-1 compliance scores, we do not warrant that any specific document will achieve full compliance or that remediation will satisfy every requirement of every regulator or auditor. You remain responsible for your own regulatory compliance obligations.
9. Limitation of Liability
To the fullest extent permitted by law, CCM Bridge’s total liability arising from these Terms or the Service will not exceed the amount you paid us in the 12 months preceding the claim, and we will not be liable for indirect, incidental, special, or consequential damages.
10. Indemnification
You agree to indemnify CCM Bridge against claims arising from Your Content (including any violation of Section 4’s document-scope restriction), your violation of these Terms, or your violation of applicable law.
11. Term and Termination
These Terms remain in effect while you have an active subscription. We may suspend or terminate access for material breach, including violation of Section 4. Upon termination, your right to use the Service ends; data deletion/export is governed by the Data Processing Agreement where applicable.
12. Governing Law
These Terms are governed by the laws of the State of Minnesota, without regard to conflict-of-laws principles. Disputes will be subject to the exclusive jurisdiction of the state and federal courts located in Minnesota.
13. Changes to These Terms
We may update these Terms from time to time. Material changes will be reflected by an updated effective date, and for active subscribers, reasonable notice will be given.
